New Behavioral Health Patients

RAHC is accepting new behavioral health patients.  The counselors see patients for depression, anxiety, trauma, women’s issues, substance abuse issues, insomnia, smoking cessation, couples counseling, children (ages 5 and up), family counseling and more.  RAHC is able to schedule new patients in less than 1 week.

RAHC also provides psychiatric services for children and adults by way of Dr. Paul Vance, a board certified tele-health provider. That means patients will have a scheduled appointment here at RAHC, but communication will be through a video conversation, like a FaceTime or Skype call made through a secure server.

To register as a new behavioral health patient, please fill out and submit the following forms:

  1. Patient Registration– one per patient
  2. Release of Information for transferring behavioral health records
  3. Sliding Fee Discount Program (if applicable)
