Physician

Rockbridge Area Health Center seeks a full-time staff physician. The Staff Physician provides patient-centered health care within a community-based setting and strives to improve general health and well-being through prevention, education, and treatment. Activities involve direct patient care, physical assessment, diagnosis and treatment, provision of preventive care, referral for specialty services as needed, and follow-up. Candidates must be board certified, or board eligible, licensed independent practitioner who devotes his/her professional services to the benefit of Rockbridge Area Health Center. RAHC provides integrated medical, dental and behavioral health care services in a new and modern facility.

Adult Care Coordinator 

The Adult Care Coordinator is responsible for assisting patients, parents/family members and caregivers to navigate the health care system by coordinating timely referrals to specialists and other care providers, facilitating communication with the individual’s primary care provider and problem solving to promote the individual’s well-being while responding to their concerns. Responsibilities include maintaining a registry of adults with complex health care needs, pre-visit planning prior to scheduled appointments, implementation of quality improvement initiatives, assist with and anticipate health care transitions between care settings, improve communication between families, the practice, community services and health care providers.

QUALIFICATIONS AND COMPETENCIES

  • Associate degree preferred
  • Licensure as a LPN by the State of Virginia
  • At least 5 years’ experience in clinical setting
  • 2 years’ experience in primary care/internal medicine setting.
  • Proof of current Basic Life Support for Healthcare Providers (BLS) certification.
  • Excellent communication and public relation skills, and strong customer service orientation. Self-directed with ability to develop well-organized systems.
  • Superb documentation skills and strong verbal and written communication skills.

Patient Care Manager 

The Patient Care Manager’s primary responsibility is to oversee triage and care management activities for the patients of the RAHC. The Patient Care Manager:

  • Works closely with patients, their families, and Care Coordinators to facilitate access to care and utilization of available care resources to manage their complex chronic illness.
  • Role is to improve health outcomes through directing care, educating patients, building trust between patients and medical practitioners and enhancing communication and continuity of care.
  • Uses a holistic, culturally sensitive approach to patient-centered care.

QUALIFICATIONS AND COMPETENCIES

  • Licensure as a Registered Nurse by the State of Virginia; BSN preferred.
  • 2 – 4 years’ experience in a clinical setting
  • Experience in a Patient Center Medical Home environment preferred
  • Proof of current Basic Life Support for Healthcare Providers (BLS) certification.
  • Demonstrated leadership abilities, excellent communication and public relations skills, and strong customer service orientation.
  • Self-directed and has ability to develop well-organized systems.
  • Supervise other team members as indicated.
