Full Time Behavioral Health Case Manager: primary responsibilities are to support and coordinate physical and mental health care for an assigned group of patients for the purpose of 1) reducing barriers to physical and mental health care 2) reducing dropout and increasing adherence to treatment 3) improving treatment outcomes and patient’s life satisfaction.
Qualifications: Bachelor’s degree from a four-year college or university or higher. 3-4 years of experience in direct patient care. Eligible to be certified as a Qualified Mental Health Professional (QMHP-E or QMHP) with the Virginia Board of Counseling.
